Requesting you repair needs using this form will priorities your repair to be completed the following day. (Any request received by 7:00 am Monday thru Friday will be done that day.) If there is no hurry and you just wish to receive an estimate please use the feedback page to do so or just call the office to schedule an appointment. 502-267-8295 Repair Information Please give as much information as possible regarding your issue. What area of the house is it on? Is your house one story or two? Is the roof walkable or steep? If it is a roof repair, what type of shingles do you have? Is there one layer or two? Any other information about the issue is important. Please include all of them in your description. Repair Fee Policy We have a $225.00 repair fee to determine the problem. Some minor repair work is included in the fee to attempt to take care of the problem. Examples of these are: Ø Nail Pops or lifting shingles Ø Re-nail loose shingles Ø Reseal vents Ø Re-seal flashing on walls and chimney All repair runs are paid at time of visit. (If you are not able to be home, a check must be taped to the back door) No repair work will be completed if payment is not available at time of service. A paid invoice for $225.00 listing the findings of the service run will be sent to your home and if applicable, a service agreement outlining what type of additional attention your home needs will be included. The service agreement of recommendations will be additional to the repair fee. There is No Guarantee or Warranty on repair work other than a repeat service run. A repeat service run will be honored for 30 days to review area if needed. If you want to complete the proposed work from Middletown, just sign the service agreement and forward it back to Middletown.
By submitting this request I agree to pay $225.00 service fee upon completion. There is no specific time for scheduling this type of repair. It will be added to the days work based on time and completion of previously scheduled repairs. |
